I like this approach, and I once did something similar to it. My university has gone to online evaluations, and students have about two weeks to complete them (if they choose to do so at all). I can cover the evaluations in class, but students decide when (or if) they will do them. With the online form, there isn't much room for reflective writing space. The questions are on a Likert scale. :-(

I like this idea! Makes for a good last day of class activity. I always struggle with topics for the last day.
